Visas
To get a visa in due time participants should submit
as soon as possible but not later that
10 August 2000 to Mrs. Elena Russakovich the following documents:
the filled-in Visa Application Form (in the Application Form),
a fax copy of the first passport pages (with the photo,
passport No and the name),
a fax copy of the document permitting one's stay/work in country,
if the visa is applied for not in one's native country
(for CERN people a copy of Attestation will do).
Application Form
The Application Form should be filled in
and submitted to the Organizing Committee (E.Russakovich) either
by E-mail or by Fax as soon as possible but not later than
10 August 2000.
